Advice to Requester
Please do not ask workers to enter any personally-identifiable information, even in the code. Asking for any part of your real name, plus your date of birth, especially for something so inconsequential as the submission code, feels very close to a violation of the TOS, and many workers will be VERY uncomfortable with doing that. It's especially egregious to ask for something like that when the hit itself asks for some very personal (though not identifiable) information.Please keep that in mind. It's best to use a code generator, easier for everyone, but if you can't or won't, just ask people to enter a random multi-digit number, or a blend of random words.
